This is a submit page template. To add it, simply add a new page and select the “Submit” page template in the “Attributes” panel. You can add your contact e-mail in the options panel under “General Settings”.

The form allows your visitors or registered users submitting new posts/galleries, then from the WordPress Dashboard you can approve the user-submitted posts/galleries to appear on your site or just delete them.
